Collection Methods 
Patient administration systems (eg: HBCIS, Cerner) have dedicated elective surgery and outpatient modules to collect this data.
Comment
Patients are able to have more than one 'not ready for care-start date' recorded within the same listing period. Some Queensland Health information systems restrict date storage to CCYYMMDD format e.g. Oracle systems.
