Code 1 Waiting
This code is used for waiting patients who do not have a future booking date for their planned service.

Code 2  Booked
This code is used for waiting patients who do have a future booking date for their planned service.

Code 3 Treated
This code is used where the planned service was performed.

Code 4 Removed
This code is used where the planned service was not performed but the patient was removed from the waiting list for another reason. Reasons the planned service was not required may include: 
-	The patient requested to be removed from the waiting list.
-	The facility removed the patient from the waiting list according to local policy, which may include: where the patient declined an offer of service; the patient failed to respond to audit measures despite reasonable efforts to contact the patient; or the patient exceeded a not ready for care time threshold for personal reasons.
-	The treating practitioner removed the patient from the waiting list for clinical reasons, such as the patient being deemed not fit for the service due to co-morbidities requiring management for stabilisation or investigation.
-	The patient died.
-	The facility ceased the service and transferred the patient on the waiting list to another facility or an alternative care pathway.
