Code FIM Functional Independence Measure (FIM)
The FIM is a basic indicator of patient disability and is used to track the changes in the functional ability of a patient during an episode of hospital rehabilitation care or geriatric evaluation and management care.

The FIM is comprised of 18 items, grouped into 2 subscales - motor and cognition.

Patient function is assessed using the FIM instrument at the start of a rehabilitation episode of care and at the end of a rehabilitation episode of care.

Code HON Health of the Nation Outcome Scale 65+ (HoNOS 65+)
The HONOS 65+ is a clinical assessment tool used to evaluate psychiatric symptoms and psychosocial functioning in an older patient during an episode of psychogeriatric care.

The HoNOS 65+ is designed to be used by clinicians before and after interventions, so that changes attributable to interventions can be measured.

The HoNOS 65+ is comprised of 12 items that rate various aspects of mental and social health.

Code RUG Resource Utilisation Groups - Activities of Daily Living (RUG - ADL)
The RUG-ADL scale measures the motor function of a patient for four activities of daily living.

Code SMM Standardised Mini-Mental State Examination (SMMSE)
The SMMSE is a clinical assessment tool which is used as a screening test for cognitive impairment.

The SMMSE consists of 12 items or questions which assess a range of cognitive domains, requiring vocal and physical actions (such as memory recall and drawing) in response to reading and listening to commands.
Mandatory
For FIM, Hospital service-care type (QHAPDC) must be code 09 Geriatric Evaluation and Management, or code 20 Rehabilitation care.
For HON, Hospital service-care type (QHAPDC) must be code 10 Psychogeriatric care.
For RUG, Hospital service-care type (QHAPDC) must be code 11 Maintenance care, or code 30 Palliative care.

Optional
For SMM, Hospital service-care type (QHAPDC) must be code 09 Geriatric Evaluation and Management.

Queensland Hospital Admitted Patient Data Collection (QHAPDC):
Must not be null for admitted patient records with Sub-acute and non-acute activity.
Validated against a list of ADL type codes.
Hospital Based Corporate Information System (HBCIS):
For operational purposes SMMSE data is accessed via a separate field 'Other ADL type'.

Queensland Hospital Admitted Patient Data Collection (QHAPDC):
Collection of SMMSE is optional.
