This code must be used for a patient removed from an elective surgery waiting list because the procedure they were awaiting had been performed as planned.
Code 2 Admitted as an emergency patient at this hospital
This code must be used for a patient removed from an elective surgery waiting list because the procedure they were awaiting had been performed as an emergency, not as planned.
Code 3 Could not be contacted
This code must be used for a patient who failed to respond to an offer for a booking or audit measures, despite reasonable efforts to contact them.
Code 4 Treated elsewhere for awaited procedure (not on behalf of this hospital or the State)
This code must be used for a patient who elected to receive their surgery privately at their own cost, or outside of Queensland.
Code 5 Surgery not required or declined
This code must be used for a patient who was removed by a designated clinician from the waiting list for clinical reasons, such as the patient being deemed not fit for the procedure due to co-morbidities requiring management for stabilisation or investigation, or removed according to local policy, or who requested to be removed from the waiting list.
Code 6 Transferred to another hospital for awaited procedure (on behalf of this hospital or the State)
This code must be used for a patient transferred off the waiting list to another public facility after the originating facility ceased the elective surgery service, patients whose co-morbidities required transfer to a facility with a higher clinical service capability, patients transferred because another facility had capacity, or those treated through the Surgery Connect program.
Code 7 Patient died before receiving awaited procedure
This code must be used for a patient who died while waiting, whether or not the cause of death was related to the condition requiring treatment.
