Collection and Usage Attributes
Code 01 Home/usual residence
Used for those patients who return to their usual residence following the current hospital stay. If the patient is usually a resident of a boarding house, commercial hostel, aged care service, independent living unit or other institution, use this category. However, if the patient is being transferred from the hospital to a residential aged care service for the first time, do not use this category; use code 15 'Residential Aged Care Service'.

Code 04 Other health care accommodation
Used for patients who are transferred to alcohol and drug centres or other health care establishments.

Code 05 Died in hospital
Used when the patient died during his/her hospitalisation.

Code 06 Care type change
Used for statistical separations where the patient is to continue the hospital stay, but is now changing the type of episode of care (e.g. Acute to Maintenance). Do not use this code for registered boarders changing status to become an admitted patient.

Code 07 Discharged at own risk
Used for patients who abscond or leave hospital against medical advice.

Code 09 Non-return from leave
Used when a patient goes on leave and does not return to the hospital within seven days. Note that the patient is to be discharged from the date that he/she left the hospital.

Code 12 Correctional facility
Used when a patient is separated to a correctional facility.

Code 13 Organ procurement
Used to denote the cessation of an organ procurement registration.

Code 14 Boarder
Used to denote the completion of a boarder registration.

Code 15 Residential aged care service
Used for patients who are discharged to a residential aged care service for the first time (i.e. the residential aged care service is not where they lived prior to their admission to hospital). A residential aged care service includes former public and private nursing homes and hostels - but not independent living units (use code 01 'Home/usual residence' for patients being returned to a residential aged care service).

Code 16 Transferred to another hospital
Used for patients who are transferred to another hospital for continuation of their admitted care and management. The second hospital undertakes full responsibility for the patient. Note that this code may be used for patients transferred to hospitals which are interstate or overseas.

Code 17 Medi-Hotel
Used for patients who are discharged to accommodation, arranged and paid for by the facility, that is used for accommodating patients post discharge where the patient is: awaiting transport; receiving on-going treatment/investigation as a non-admitted patient (includes minimal (low) care nursing) or receiving a course of treatment (such as chemotherapy) and requires accommodation close to the hospital between treatments.

Code 19 Other
Used for patients who are separated under circumstances that do not fit any other category. It is expected this code will be rarely used.

Code 31 Residential mental health care facility
Used for patients transferred to a residential mental health care facility providing treatment in a community based setting, including Community Care Units, Step Up Step Down Units and Youth Residential Rehabilitation Units.

Code 32 Change of reference period
Used for statistical separations to signify the end of a reference period. This code should only be used at the end of the reference period at 11:59 on the 30/06/YYYY. Currently this code is only valid in residential mental health care facilities.
Queensland Hospital Admitted Patient Data Collection (QHAPDC):
Must not be null.
Validated against a list of mode of separation codes.
Queensland Hospital Admitted Patient Data Collection (QHAPDC):
Particular care should be taken when entering mode of separation codes for patients being transferred to another facility. Incorrect code application may affect Queensland Health's ability to obtain funding for services provided to compensable, entitled veterans, and/or defence force personnel in relation to Queensland Ambulance Service (QAS) inter-facility transfers.

For reporting of the national data element, Episode of admitted patient care-separation mode (NHDD), the following mapping occurs:
Code 01 Home/usual residence is mapped to code 9 Other (includes discharge to usual residence, own accommodation/welfare institution (includes prisons, hostels and group homes providing primarily welfare services)).
Code 04 Other health care accommodation is mapped to code 4 Discharge/transfer to other health care accommodation (includes mothercraft hospitals).
Code 05 Died in Hospital is mapped to code 8 Died.
Code 06 Care type change is mapped to code 5 Statistical discharge - type change.
Code 07 Discharged at own risk is mapped to code 6 Left against medical advice/discharge at own risk.
Code 09 Non return from leave is mapped to code 7 Statistical discharge from leave.
Code 12 Correctional facility is mapped to code 9 Other (includes discharge to usual residence, own accommodation/welfare institution (includes prisons, hostels and group homes providing primarily welfare services)).
Code 13 Organ Procurement is mapped to code 9 Other (includes discharge to usual residence, own accommodation/welfare institution (includes prisons, hostels and group homes providing primarily welfare services)).
Code 14 Boarder is mapped to code 9 Other (includes discharge to usual residence, own accommodation/welfare institution (includes prisons, hostels and group homes providing primarily welfare services)).
