Collection and Usage Attributes
Queensland Hospital Admitted Patient Data Collection (QHAPDC):
Must not be null for admitted patient records where Episode of admitted patient care-compensable patient status is code 1 (Workers' Compensation (Queensland)).
Must not be after the separation date.
Record the incident date using the full date (i.e. DDMMYYYY) and leading zeros where necessary.

Although provision is made for recording estimates of the day and month of the incident date, every effort should be made during the course of the admission to determine (and record) the actual incident date.

Hospital Based Corporate Information System (HBCIS):
If any part of the incident date is not known, use the below conventions:
- If the day of the incident is unknown, use ** for the first two values.
- If the month of the incident is unknown, use ** for the third and fourth values
- If the year of the incident is unknown, an estimate must be provided.

For paper/excel form submissions, date values must be input in dd/mm/yyyy format. If any part of the incident date is not known, use the below conventions:
- If the day of the incident is unknown, use 15 for the first two values.
- If the month of the incident is unknown, use 06 for the third and fourth values
- If the year of the incident is unknown, an estimate must be provided.

Public hospitals that do not use an electronic patient management system will need to complete a Motor and Work Injury Interview Questionnaire and provide this information to the facility that records their data.
The workers' compensation data elements are required for a patient who is a Queensland worker with a compensable injury or illness who has elected to be treated as a public patient by a doctor nominated by the hospital, and are used to substantiate a hospital's claim for reimbursement for costs of providing treatment to a patient for compensable injuries.
