This code must be used for a patient removed from an elective surgery waiting list because the procedure they were awaiting had been performed as planned. Includes patients undergoing the awaited procedure whilst admitted for another reason.

Code 02 Admitted as an emergency patient for awaited procedure at this hospital
This code must be used for a patient removed from an elective surgery waiting list because the procedure they were awaiting had been performed as an emergency, not as planned. Admission as an emergency patient could be due to other causes such as the condition requiring treatment deteriorated whilst waiting, inappropriate urgency rating, delays in the system, or unpredicted biological variation.

Code 03 Could not be contacted
This code must be used for a patient who failed to respond to an offer for a booking or audit measures, despite reasonable efforts to contact them. Includes patients who have died while waiting whether or not the cause of death was related to the condition requiring treatment.

Code 04 Treated elsewhere for awaited procedure (not on behalf on this hospital or state/territory)
This code must be used for a patient who elected to receive their surgery privately at their own cost, or outside of Queensland.

Code 05 Surgery not required or declined
This code must be used for a patient who was removed by a designated clinician from the waiting list for clinical reasons, such as the patient being deemed not fit for the procedure due to co-morbidities requiring management for stabilisation or investigation, or removed according to local policy, or who requested to be removed from the waiting list.

Code 06 Transferred to another hospital for awaited procedure (on behalf on this hospital or state/territory)
This code must be used for a patient transferred off the waiting list to another public facility after the originating facility ceased the elective surgery service, patients whose co-morbidities required transfer to a facility with a higher clinical service capability, patients transferred because another facility had capacity, or those treated through the Surgery Connect program.

The waiting time on the waiting lists at the initial hospital and subsequent hospitals should be combined for national reporting.

Queensland Hospital Admitted Patient Data Collection (QHAPDC):
This data element is only supplied by public hospital services.
Routine admission for the awaited procedure is only one reason why patients are removed from the waiting list. Each reason for removal provides different information. These data are necessary to augment census and throughput data. For example, after an audit the numbers of patients on a list would be expected to reduce. If an audit were undertaken immediately prior to a census the numbers on the list may appear low and not in keeping with the number of additions to the list and patients admitted from the list.
