Collection and Usage Attributes
Post office box numbers/mail service numbers shall not be recorded. Use a building/property number (or rural property name if applicable) and street name wherever possible. Rural properties have access roads which may be named.

Example:
"Emohruo Homestead", Dusty Road

Use the postcode as an indicator. For example, If the person states the address is "Emohruo Homestead", off Dusty Road, Elbow Valley, via Warwick, and both Elbow Valley and Warwick have postcodes, record the property name and access road name as the address of usual residence. Record ELBOW VALLEY as the suburb/town/locality of usual residence. Do not record the word "via".

If Elbow Valley had no postcode, then the property name and the access road shall be recorded as the address of usual residence. Record WARWICK as the suburb/town/locality of usual residence. Do not record Elbow Valley.

Consideration shall be given to the unnecessary use of non-alphanumeric characters and symbols, which should be avoided wherever possible due to implications on system functionality.
Queensland Hospital Admitted Patient Data Collection (QHAPDC) and Queensland Perinatal Data Collection (QPDC):
Must not contain unidentified characters/symbols.
Left justified and space filled to 40 characters; may be blank.
Usual residence is the place where a person usually lives (resides). It is not the residence where a person might stay temporarily before or after a period of hospitalisation/treatment/contact.

Person's without a permanent address that are currently living within a residential mental health care facility should have the address of that facility recorded as their usual residence.

Interstate and overseas persons:
It is particularly important to record the correct address for persons who live interstate or overseas. This is for follow up and to support funds that may be transferred between state health departments for patients who are treated outside their state of usual residence.

Baby for adoption:
The Department of Communities, Child Safety and Disability Services or the foster carer will advise the relevant QH facility with regard to the correct address details for correspondence from QH during the transitional period. This will usually be either the foster carer or the Department.

For Hospital Based Corporate Information System (HBCIS) this data is captured from the "Address Line" where the "Address Type" value is equal to code P (Permanent).

Queensland Perinatal Data Collection (QPDC):
Record the number and street name of usual residence of the mother.

Residential Mental Health Care Data Collection (RMHCDC):
In the absence of a home address (i.e. the consumer is awaiting public housing) the address of usual residence may be reported as the address of the residential mental health care facility for episodes where the consumer stay extends across reporting periods (financial year), or for a transfer to another facility where a consumer is expected to return to this facility within 42 days.

Where the consumer episode of residential mental health care is ended (formally discharged), the address of usual residence should be the address to which the consumer is going.
The collection of the address details is critical for patient follow up. It is also used with other data elements to determine the Statistical Area of usual residence. This enables:
- comparison of the use of services by persons residing in different geographical areas,
- characterisation of catchment areas and populations for facilities for planning purposes, and
- documentation of the provision of services to residents of states or territories other than Queensland.
