Guide for Use
A practitioner may have more than one given name.
A practitioner's full given name(s) should be recorded. Where applicable it is essential that the given names are recorded for the first 3-recorded given names of a practitioner and desirable for the fourth and subsequent given names.
Mixed case should be used.
Given name(s) (first and other given names) may be recorded in one field or several fields. If recorded in the one field a delimiter should be used between each given name. This metadata item definition applies regardless of the format of data recording.
A full history of given names is to be retained.

Collection Methods
Use of first initial
If the health care client's given name is not known, but the first letter (initial) of the given name is known, record the first letter as the given name. Do not record a full stop following the initial.
Clients with only one name
Some people do not have a family name and a given name and they have only one name by which they are known. If the practitioner has only one name, record it as Family name.
Punctuation
If special characters form part of the given name, consideration should be given to a system's capability to collect and interpret such characters.
Hyphen
Do not leave a space before or after the hyphen, i.e. between last letter of 'Anne' and the hyphen, nor a space between the hyphen and the first letter of 'Maree', e.g. Anne-Maree.
Spaces
If the client has recorded their given name as more than one word, displaying spaces in between the words, record their given names in data collection systems in the same way, e.g. Jean Claude.
Ethnic names
The Appendix C of the Australian Standard AS5017-2006 Health Care Client Identification or the Centrelink publication A Guide to Ethnic Naming Practices, provide information on the naming practices of people from culturally diverse backgrounds.
Application for approval as a patient-class prescriber Data Supply Requirement (DSR):
This information is collected using the Application for Approval as a Patient-class prescriber form.
Provide the given names of the prescriber applying for approval in section 1 of Application for Approval as a Patient-class prescriber form.
In the case of unexpected leave or illness,a prescriber who can temporarily treat patients of the prescriber who is applying for approval should be nominated. Provide the given names of the supporting prescriber in section 3 and 4 of the If the prescriber works in more than one practice then the practice name and address details should be provided for each practice location for question.
