Guide for Use
Date of incident should be recorded when the injury or illness for which the patient is treated appears to have been caused as a result of either:
- working for an income; or
- a road traffic accident;
regardless of the compensable status of the patient at the time of admission.
New
Verification Rules
Queensland Hospital Admitted Patient Data Collection (QHAPDC):
Must not be in the future (ie. past current date).
Must be on or before the admission date.
Can be null.
Collection Methods
Queensland Hospital Admitted Patient Data Collection (QHAPDC):
When a patient is being registered at a hospital for treatment, ask one of the following questions:
- Following an accident or injury, ask the patient "On what date did the accident or injury occur?"
- In the case of late onset of injury or illness, ask the patient "On what date were you first assessed by a doctor or dentist for this injury or illness?"
Record the incident date using the full date (i.e. DDMMYYYY) and leading zeros where necessary.
Although provision is made for recording estimates of the day and month of the incident date, every effort should be made during the course of the admission to determine (and record) the actual incident date.
Hospital Based Corporate Information System (HBCIS):
For HBCIS Hospitals, the input display will be in DDMMYYYY format. If any part of the incident date is not known, use the below conventions:
- If the day of the incident is unknown, use ** for the first two values.
- If the month of the incident is unknown, use ** for the third and fourth values
- If the year of the incident is unknown, an estimate must be provided.
Other system(s):
For electronic submissions to the QHAPDC, date values must be transmitted in CTYYMMDD format (see Episode of admitted patient care-date of incident (QHAPDC DSR) data element). For paper/excel form submissions, date values must be input in dd/mm/yyyy format. If any part of the incident date is not known, use the below conventions:
- If the day of the incident is unknown, use 15 for the first two values.
- If the month of the incident is unknown, use 06 for the third and fourth values
- If the year of the incident is unknown, an estimate must be provided.
Comments
Assists in the validation of a patient's hospital treatment against any claims they may make for compensation with WorkCover Queensland or the Motor Accident Insurance Commission.
