* Important – Before making any modifications it is important to check via The Viewer that a patient is not admitted to any facility as there may be implications for the patient’s clinical record.

Record the date of birth of the person using the full date and leading zeros where necessary.

For data collections concerned with children's services, it is suggested that the estimated date of birth of children aged under two years should be reported to the nearest three month period, i.e. 0101, 0104, 0107, 0110 of the estimated year of birth. For example, a child who is thought to be aged 18 months in October of one year would have his/her estimated date of birth reported as 0104 of the previous year. Again, an estimated date flag or date accuracy indicator should be reported in conjunction with all estimated dates of birth.

Although provision is made for recording an unknown date of birth every effort should be made to determine and record the person's actual date of birth or best estimate of date of birth.

Death Registration Data Supply Requirement (DSR):
This data element relates to the deceased person’s date of birth.
Shall not be null.
In data collections that also record date of death for the same person, date of birth must be less than or equal to date of death.
Information on date of birth can be collected using the one question:

'What is your/ (the client's) date of birth?'

In self-reported data collections, it is recommended that the following response format is used:

Date of birth: _ _ / _ _ _/ _ _ _ _

The linking of client records from diverse sources, the sharing of client data, and data analysis for research and planning all rely heavily on the accuracy and integrity of the collected data. In order to maintain data integrity and the greatest possible accuracy an indication of the accuracy of the date collected is critical. For this reason it is strongly recommended that the data element Estimated date of birth flag also be recorded at the time of record creation to flag the accuracy of the data.

HBCIS:
- If the day of birth is unknown, use **
- If the month of birth is unknown, use **
- If the year of birth is unknown, estimate the year from the age of the patient

Although provision is made for recording an estimated date of birth (using **/**/YYYY), every effort should be made during the course of the admission to determine (and record) the patient's actual date of birth. The patient's date of birth is an important requirement for the correct identification of the individual and the accurate assignment of a Diagnosis Related Group (DRG) at a later date.

Queensland Perinatal Data Collection (QPDC):
Date of birth is collected for both mother and baby.

Queensland Opioid Treatment Program (QOTP) Admission Data Supply Requirement (DSR), Queensland Opioid Treatment Program (QOTP) Discharge Data Supply Requirement (DSR), data is collected by completing the Queensland Opioid Treatment Program (QOTP) Admission/Discharge form. Provide the client's date of birth as indicated on the Queensland Opioid Treatment Program (QOTP) Admission/Discharge form.

Queensland Trauma Data Collection:
ATR validation: less than all other dates
ATR correspondence: single
If date of birth is not known or cannot be obtained, provision should be made to collect or estimate age.
If year of birth is known (but date of birth is not) use the date, 0101YYYY of the birth year to estimate age (where YYYY is the year of birth).
If person is aged under 2 years, date of birth should be estimated to the nearest three month period, ie 0101, 0104, 0107 or 0110 of the estimated year of birth.

Date of birth enables derivation of age for use in demographic analyses, assists in the unique identification of clients if other identifying information is missing or in question, and may be required for the derivation of other data elements (e.g. diagnosis related group for admitted patients). Privacy issues need to be taken account in asking clients their date of birth. Wherever possible and wherever appropriate, Date of birth should be used rather than Age because the actual date of birth allows more precise calculation of age. The representation layout of a date may vary depending on the purpose. For the recommended display of a date, please refer to the Display of Date and Time in Electronic Systems DSS. The input, storage, messaging or reporting of a date may be done in other formats, if supported by business rules and standards.
