Collection and Usage Attributes
Quality Improvement Payment (QIP) incentive payments are offered by the Department of Health to improve the number of public hospital inpatients offered the Smoking Cessation Clinical Pathway. The Smoking Cessation Clinical Pathway form was developed by the Statewide Respiratory Clinical Network and is an evidence based decision support tool to aid health staff in assisting patients to quit smoking tobacco.

Clinicians (e.g. nurses, pharmacists, allied health practitioners, and medical officers) are responsible for completing the Smoking Cessation Clinical Pathway form and ensuring it is filed in the patient's medical record prior to separation.

Clinical coders assess if filed Smoking Cessation Clinical Pathways are deemed complete by reviewing the grey sections on page 1 only. A completed Smoking Cessation Clinical Pathway equates to documented evidence of the following criteria:
- for identified smokers (question 1 = Yes) at least one box must be marked in each grey section (i.e. for each of questions 4, 8 and 11), and
- all four columns in the Signature Log (name, designation, signature and date) must be completed.

Code N No
None of the mandatory items on a Smoking Cessation Clinical Pathway form have been completed for the patient who self-identifies as being a current smoker of tobacco, or no pathway is available within the patient record.

Code P Partial
Some of the mandatory items on a Smoking Cessation Clinical Pathway form have been completed for the patient who self-identifies as being a current smoker of tobacco.

Code Y Yes
The mandatory items and signature log on a Smoking Cessation Clinical Pathway form have been completed for the patient who self-identifies as being a current smoker of tobacco.

Must be null when Episode of admitted patient care-tobacco smoking status of patient is code 2 (Reported not a current smoker) or code 9 (No smoking status reported or documented).
Hospital Based Corporate Information System (HBCIS):
- If the Smoking Status and Smoking pathway completed? (Y/P/N) fields are populated and ICD-10-AM code Z72.0 is removed from the list of diagnoses, the two fields are reset to null (blank).
- If the Smoking Status and Smoking pathway completed? (Y/P/N) fields are populated and ICD-10-AM code Z72.0 is removed from the list of diagnoses and ICD-10-AM code Z86.43 remains, the Smoking Status field is auto-populated with the value '2' and the Smoking pathway completed? (Y/P/N) field is reset to null (blank).
The introduction of code 'P' is valid for separations on or after 1 July 2016.
Note: v5.00 of the Smoking Cessation Clinical Pathway form dated 02/2017 must be used for QIP: https://www.health.qld.gov.au/improvement/pathways/docs/smoking-pathway.pdf.
