1. Licensed Private Freestanding Day Surgery Centre
2. Licensed Private Day Centre/Hospital
3. Licensed Private Hospital / Acute other
33. Recognised Public Hospital / Acute outpost
34. Recognised Public Hospital / Acute other
36. Public Psychiatric Residential Facility

To be recorded by contracted hospitals (Type B, i.e. the provider of the hospital service) where the service involves inter-hospital contracted care.

Must be collected by contracted hospitals (where Contract Role = Type B) and the Contract Type is 2, 3, 4 or 5 (inter-hospital contracted care).
