Functional Independence Measure (FIM):
The FIM tool is a basic indicator of patient disability and is used to track the changes in the functional ability of a patient during an episode of hospital rehabilitation care.

Patient function is assessed using the FIM tool at the start of a rehabilitation episode of care and at the end of a rehabilitation episode of care. Admission assessment is collected within 72 hours of the start of a rehabilitation episode. Discharge assessment is collected within 72 hours prior to the end of a rehabilitation episode.

Health of the Nation Outcome Scale 65+ (HoNOS 65+):
The HONOS 65+ tool is a clinical assessment tool used by mental health professionals to evaluate psychiatric symptoms and psychosocial functioning in an older patient.

HoNOS 65+ is designed to be used by clinicians before and after interventions, so that changes attributable to interventions can be measured.

Resource Utilisation Groups - Activities of Daily Living (RUG - ADL):
The RUG-ADL tool measures the motor function of a patient for 4 activities of daily living. RUG-ADL is a 4 item scale measuring patient motor function for activities of daily living.
Hospital service-care type (QHAPDC) must be code 09 Geriatric Evaluation and Management, 10 Psychogeriatric care, 11 Maintenance care, 20 Rehabilitation care or 30 Palliative care.
