The date that a request for service is received by an outpatient clinic may or may not be the same as the date the request is entered onto the outpatient scheduling system.

In the case of a self-referral (other request), the date the request for service is received is the date the person first made contact with the clinic.
Must not be earlier than the Outpatient clinic written referral-issue date.
The representation layout for the date may vary depending on the purpose. Date input, storage, messaging or reporting may be done in other formats, if supported by business rules. For the recommended display of a date, please refer to the Display of Date and Time in Electronic Systems DSS.
