Collection and Usage Attributes
Queensland Hospital Admitted Patient Data Collection (QHAPDC) and Queensland Perinatal Data Collection (QPDC):
Validated against the CRDS Locality Data Set.
Left adjusted and space filled to 40 characters.
Must not be blank.
Usual residence is the place where a person usually lives (resides). It is not the residence where a person might stay temporarily before or after a period of hospitalisation/treatment/contact.

The Locality (suburb) line of the address shall not include a state code if the patient lives in Australia. It shall not include the word "via". For example, Home Hill via Ayr - the Locality shall be reported as HOME HILL. For localities with two names, ensure there is only one character space between the names.

Interstate and overseas persons:
It is particularly important to record the correct address for persons who live interstate or overseas. This is for follow up and to support funds that may be transferred between state health departments for patients who are treated outside their state of usual residence.

Unknown locality:
Record UNKNOWN (e.g. an unconscious person is unable to provide the information). However, every effort should be made during the course of the admission to determine (and record) a person's locality details. Do not leave the field blank.

Baby for adoption:
The Department of Child Safety, Youth and Women or the foster carer will advise the relevant Queensland Health facility with regard to the correct address details for correspondence from Queensland Health during the transitional period.

Record AT SEA.

Queensland Perinatal Data Collection (QPDC):
Record the suburb/town/locality of usual residence of the mother.
The collection of the address details is critical for patient follow up. It is also used with other data elements to determine the Statistical Area of usual residence. This enables:
- comparison of the use of services by persons residing in different geographical areas,
- characterisation of catchment areas and populations for facilities for planning purposes, and
- documentation of the provision of services to residents of states or territories other than Queensland.
Statistical Services Branch (SSB) maintains a corporate standardised data set of localities, postcodes, states and associated Statistical Area level 2 (SA2) identifiers. The content of the locality data set has two main sources, the Australian Bureau of Statistics (ABS) and Australia Post, with supplementary data supplied by Queensland Health.
The CRDS file can be browsed in QHEPS via INTERNAL QH LINK NOT AVAILABLE or an extract can be requested via email crds@health.qld.gov.au.
